To better serve our students, Clarkson College partners with CastleBranch to manage our students' health and safety compliance online. CastleBranch allows us to meet the needs of the clinical agencies and/or accrediting bodies’ clinical requirements. Students with a clinical experience in their academic program will place an order using package codes that have been aligned to each academic program. Package codes will be e-mailed to student’s College e-mail account. Once your myCB account is created and opened, your appropriate requirements (To-Do Lists) will be visible. Using myCB students will maintain and submit documentation directly to their account.

Immunizations

If you were born before 1957, this requirement is waived. Submit documentation of one of the following:

  • Two vaccinations
  • Positive antibody titers for Measles, Mumps and Rubella (lab report or physician verification of titer results required)

Submit documentation of one of the following:

  • Two vaccinations (vaccine has been available since 1995)
  • Positive antibody titer (lab report or physician verification of titer results required)

Submit documentation of one of the following:

  • If record of the Hep B series (three immunizations), submit series as well as results of a positive Hepatitis B Surface Antibody (HBsAB) titer (lab report of physician verification of titer results required)
    • If the HBsAB titer is negative (non-reactive), the student will be instructed on precautions and no further immunizations are required
  • If no record of the Hep B series, a titer may be drawn and results resubmitted.

NOTE: Clarkson College policy states that if the Hepatitis B titer is non-reactive and there is no record of a previous Hep B series, the student must receive the series of Hepatitis B immunizations followed by a second titer. If the second titer is non-reactive, no further immunizations are required.

Submit documentation of one of the following for negative TB test:

  • Results of TB skin tests (upon admission and annually thereafter)
  • Results of TB blood tests (IGRAs: QuantiFERON®–TB Gold In-Tube test (QFT-GIT), T-SPOT®.TB test (T-Spot))

Submit documentation of one of the following if positive TB test:

  • Chest X-ray (lab report required)
  • Results of TB blood tests (IGRAs: QuantiFERON®–TB Gold In-Tube test (QFT-GIT), T-SPOT®.TB test (T-Spot))
  • Completed TB questionnaire signed by health care provider. Questionnaire is available to download from this requirement.

Submit documentation of a TDaP received within the past 10 years. TD is acceptable upon renewal.

Submit documentation of a flu shot administered during the current flu season or a declination waiver. If submitting a declination waiver, you must follow up with your program director. Due by Nov. 30 each year. Documentation must be labeled for the appropriate flu season; for example, “2014-2015 flu season". The appropriate flu season MUST BE handwritten or typed on the document being submitted. The cutoff is July 31 for the new season. This is to standardize and ensure that the proper season is being reviewed and processed.

Submit documentation only if living in campus housing.


Background Check & Drug Test

Personal information requested: full name, date of birth, social security number, current address, phone number and e-mail. E-mail address must be Clarkson College e-mail. Certified Profile will run the background check from the entered demographic information and post a report to your account to complete.

Download, print and complete the Nebraska DHHS Abuse Form.

Submit documentation:

  • Form, must be complete, if not applicable please use NA in the box.
  • Form, must be faxed or scanned and notarized.
  • Complete step 1 only. Step 2 will be completed by Certified Profile. Certified Profile will receive a report and post to your account to complete.

After placing an order, students will receive an e-mail directly from the lab containing an electronic chain of custody (echain) within 24-48 hours. The subject of this e-mail will read “Form Fox,” and it will explain where you need to complete the drug test. If the e-mail is not received, contact Certified Profile at 1.888.914.7279 and have this resent. Certified Profile will receive a report and post to your account to complete.


Additional Requirements

Must be the American Heart Association Basic Life Support for Providers course. For proper documentation, submit a copy of the card’s front and back sides. The card must also be signed.

Clarkson College Basic Life Support Course Information
The course length is a maximum of four hours. Classes are offered on selected Saturdays and weekdays (morning, afternoon and evening classes). Classes are filled on a first come, first served basis and fill quickly. Pre-registration is required. Requirement for Nurse Anesthesia students only.
The following items are required:

  • Courses must be the American Heart Association Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) course
  • Submit a copy of the card’s front and back sides. The card must also be signed.
  • Submit documentation of your current licensure or verification of licensure from your state’s website.
  • Submit a copy of your current American Registry of Radiologic Technologist (AART) card or verification certification from the ARRT website.

Depending on the academic program, this may be required or optional. If optional, the requirement will cycle every four months to be answered “yes” or “no.” If required by clinical site attending, download, print and complete the Health & Safety Physical Examination Form and submit documentation to this requirement. The form must be signed by a health care provider.

Submit a copy of your current health insurance card or proof of coverage. If submitting as an active duty member or retiree and you have a TRICARE Card, you must submit your card along with letter of eligibility from your carrier (a letter from DEERS is acceptable). The contact number for DEERS is 800.538.9552 and/or TriWest is 1.877.988.9378. For non-military insurance, a letter of eligibility from your carrier is also required if you name is not listed on the card.

Download, print and complete the Clinical FERPA Release Form and submit. This will be completed annually and must be physically signed. Due by Nov. 30 each year.

If required, download, print, and complete the Confidentiality Statement and submit. Form must be witnessed by someone older than 19.
