Students admitted to programs that include a clinical component at a health care facility may be required to submit to a criminal background and/or drug screening. Students participating in other degree or service experiences may also be screened (background check/drug screening) upon request of the agency.
The screening(s) will be at the expense of the student. Additional background checks and/or drug screening could be required with or without cause for suspicion as required by the clinical agency or Clarkson College and at the expense of the student.

Drug screenings are intended for students admitted to programs that include a clinical component at a health care facility. A driver's license and Chain of Custody form are required at the diagnostic center locations to complete the drug screening.
The drug screening requires a Chain of Custody form to be completed. Various Omaha diagnostic center locations have forms available onsite, which are noted below. Other diagnostic center locations may offer an electronic form to be submitted instead. Contact Sharon Mantz to see if this option is available for you.
Nebraska residents will receive an e-mail from applicantrelease@hirease.com with "Applicant Release Form" in the subject line. The DHHS submits your information to check the adult and child abuse registries. To complete this process, select "Applicant information request" in the e-mail.

Note: Testing Centers may require an appointment (not stated on labs' website). Please call ahead to schedule an appointment.
Hours displayed are regular business hours; variations may occur due to holidays.
* No Chain of Custody form needed—location has forms are on-site. Students will state that they are a Clarkson College student at time of appointment.

Students are responsible for the cost of the drug screen, which is ordered at the same time as the background check or separately by using designated logins. When taking the drug screen at a diagnostic location, students do not pay any additional fees. If the diagnostic location discusses a fee, it should be billed to Hirease, Inc.
